Continuum of Care Model
The HSE has developed a 4-tier, person-centred, model of rehabilitation based on the
principle of a “continuum of care” that allows the individual to access the range of supports
they need to achieve their personal recovery goals in line with their needs and aspirations.
This model of care is designed to enable people to receive the support they need as close
to home as possible, and at the level of complexity that best corresponds to their needs
and specific circumstances. The various types of intervention and the settings in which
they are provided are set out below.
